At our site we give the muster captain a small card with 3 randomized injects (“radio down,” “missing headcount,” “blocked haul”) and they pick one mid‑drill so crews can’t script it, @imoore62. We cap it at 4 minutes so it stays useful — not dinner theater.

Quick add to @chloe16’s injects: we rotate the active muster point unannounced and the first person there has to “call the secondary egress and nearest refuge” out loud, or we redo the drill — keeps eyes up without turning it into a scavenger hunt. Works well, but we post the day’s map for contractors so nobody gets spun around.
